Veltri, Inc. recognizes eleven (11) paid holidays for drivers: New Year’s Day Martin Luther King Day President’s Day Memorial Day Juneteenth Independence Day Labor Day Columbus Day Veteran’s Day Thanksgiving Day Christmas Day Vacation Every employee is eligible for vacation pay after 1 year of service otherwise known as their anniversary date. Employees with 2080 hours accumulated upon their anniversary date are eligible for After one (1) year – 2 Weeks After ten (10) years – 3 Weeks Driver Benefits Your health and future are important and we support you with more than just words. Veltri acknowledges our drivers are working hard for the present, as well as building prosperity for the future. We offer superior health, dental, disability, and life insurance plans. To demonstrate our support for our drivers, Veltri pays for health care for our drivers (family coverage is additional). Furthermore, we believe your future is important and we contribute 5% to your 401K. Health Insurance Once your application for insurance has been accepted, coverage will begin on the first day of the month following 90 days of employment. Brief Description of Coverage: The plan provides comprehensive hospital and major medical insurance. Participants can choose to go to PPO or non-PPO providers of medical services by using a PPO provider. Participants will pay a lower percentage of the co-insurance, lower deductible, and a lower annual maximum co-insurance amount. Prescription Drug Benefits: The company provides a prescription drug card plan as part of the health benefits program. Dental Insurance Once your application for insurance has been accepted, coverage will begin on the first day of the month following 90 days of employment. Brief Description of Coverage: Participants will pay a percentage of the coinsurance and deductible based on the type of procedure performed. Life Insurance Once your application for insurance has been accepted, coverage will begin on the first day of the month following one calendar month of employment after hire. Brief Description of Coverage: Employees will be entitled to life insurance coverage with a face value of $15,000. This plan also offers an accelerated life benefit pay in the event of a terminal illness and the ability to purchase additional voluntary insurance for themselves or dependents. Short-Term Disability Insurance Employees are entitled to short-term disability benefits the first of the month following one year of service. Brief Description of Coverage: Disability coverage is for non-work related sickness or injury. The short-term disability plan has a maximum benefit period for a limited number of weeks, subject to a 30 day elimination period. The weekly benefit amount is generally 60% of the employee’s weekly wage/salary, subject to a maximum weekly limit. Supplemental Life/AD&D-Voluntary – Plan through Sun Life Financial Additional life insurance is available in $15,000 units up to a maximum of $60,000 Participants in supplemental life/AD&D may also purchase dependent coverage Portable or Convertible Vision Insurance – Plan through VSP Employees are entitled to Vision benefits the first of the month following the date of hire. Brief Description of Coverage: This plan offers access to care from a VSP wellness doctor, quality eyewear, and affordability at low out of pocket cost. The plan offers at no or low cost a yearly wellness exam, eyeglass frames, eyeglass lenses, and contact lenses. Flexible Benefits The employee’s contribution for health, vision, and dental coverage will be deducted from the employee’s paycheck on a pre-tax basis. This pre-tax advantage is made possible by Section 125 of the Internal Revenue Code. 401(k) Plan (company contributes 5% not vesting period) This component allows the employee to set aside pre-tax dollars, via payroll to a retirement account. Veltri Inc. contributes 5% of your gross pay to your 401 k savings plan.
